Indications

  • Treatment of chronic iron overload
  • Chronic iron overload due to blood transfusions
  • Thalassaemia treatment
  • Sickle cell disease treatment
  • Myelodysplastic syndrome treatment
  • Non-transfusion-dependent thalassemia syndrome treatment

Pharmacology

  • Orally active chelator
  • Highly selective for iron
  • Tridentate ligand
  • Binds iron in 2:1 ratio
  • Promotes excretion of iron
  • Low affinity for zinc and copper
  • Iron excretion study in thalassaemia patients

Dosage

  • Initiation after transfusion of 20 units of packed red blood cells
  • Initial daily dose of 20-40 mg/kg
  • Serum ferritin monitoring every month
  • Dose adjustment based on serum ferritin trends

Administration

  • Once daily on empty stomach
  • Tablets dispersed in water or juice
  • Must not be chewed or swallowed whole
  • Not to be taken with carbonated drinks or milk

Interaction

  • Avoid with aluminium-containing antacids
  • Caution with CYP3A4 metabolized drugs
  • Consider dose increase with concomitant use of specific drugs
  • Interaction with drugs with ulcerogenic potential
  • Caution with anticoagulants

Contraindications

  • Hypersensitivity to active substance or excipients
  • Creatinine clearance less than 40 ml/min
  • Serum creatinine >2 times upper limit of normal

Side Effects

  • Cytopenias
  • Gastrointestinal effects
  • Headache and dizziness
  • Elevation of hepatic transaminases
  • Stevens-Johnson syndrome
  • Auditory and ocular disturbances

Pregnancy & Lactation

  • Pregnancy Category C
  • No adequate studies in pregnant women
  • Possible excretion in human milk

Precautions & Warnings

  • Gastrointestinal bleeding and ulceration
  • Caution with drugs having ulcerogenic potential
  • Monitoring blood counts
  • Serious hypersensitivity reactions

Use in Special Populations

  • Pediatric use similar to adult use
  • Elderly patients at increased risk
  • Not studied in patients with renal or hepatic impairment

Overdose Effects

  • Maximal dose of 40 mg/kg
  • No information about overdose
