Indications
- Treatment of chronic iron overload due to blood transfusions
- Treatment for thalassaemia
- Treatment for sickle cell disease
- Treatment for myelodysplastic syndrome
- Treatment for non-transfusion-dependent thalassemia syndromes
Pharmacology
- Highly selective chelator for iron
- Binds iron with high affinity
- Promotes excretion of iron in the faeces
- Low affinity for zinc and copper
Dosage
- Initial daily dose of 20-40 mg/kg body weight
- To be taken on an empty stomach
- Tablets dispersed in water or juice
- Serum ferritin to be monitored every month
Administration
- To be taken once daily on an empty stomach
- Tablets dispersed in water or juice
- Not to be taken with carbonated drinks or milk
Interaction
- Not to be taken with aluminium-containing antacids
- Caution with drugs metabolized through CYP3A4
- Dose adjustments with concomitant use of certain drugs
- Caution with drugs with ulcerogenic potential or anticoagulants
Contraindications
- Hypersensitivity to the active substance or excipients
- Creatinine clearance less than 40 ml/min
- Serum creatinine >2 times the upper limit of normal
Side Effects
- Cytopenias
- Gastrointestinal effects (diarrhea, constipation, vomiting, etc.)
- Abdominal pain, distension, dyspepsia
- Headache and dizziness
- Elevation of hepatic transaminases
- Stevens-Johnson syndrome and skin rash
- Auditory and ocular disturbances
Pregnancy & Lactation
- Pregnancy Category C
- Caution in pregnancy due to potential risk to the fetus
- Not known if excreted in human milk
Precautions & Warnings
- Gastrointestinal bleeding and ulceration may occur
- Cytopenias including agranulocytosis and neutropenia
- Serious hypersensitivity reactions reported
Use in Special Populations
- Similar safety and efficacy in pediatric patients
- Elderly patients at increased risk for toxicity
- Not studied in patients with renal or hepatic impairment
