Indications
- Treatment of chronic iron overload due to repeated blood transfusions in adult and pediatric patients aged 2 years and over with thalassaemia, sickle cell disease, and myelodysplastic syndrome
- Treating chronic iron overload in patients with non-transfusion-dependent thalassemia syndromes aged 10 years and older
Pharmacology
- Orally active chelator
- Highly selective for iron
- Tridentate ligand that binds iron with high affinity in a 2:1 ratio
- Promotes excretion of iron in faeces
- Low affinity for zinc and copper
- Results in mean net excretion of iron in iron overloaded patients
Dosage
- Recommended initial daily dose of 20-40 mg/kg body weight per day
- Monitored serum ferritin every month
- Adjust dose every 3 to 6 months based on trends in serum ferritin
- Up to 40 mg/kg doses considered if iron overload is inadequately controlled
- Dose reduction steps to maintain serum ferritin at 500 microgram/l
- Dose interruption if sustained control is achieved
Administration
- To be taken once daily on an empty stomach at least 30 minutes before food
- Tablets dispersed in a glass of water or apple/orange juice
- Dispersion in carbonated drinks or milk not recommended
Interaction
- Avoid with aluminium-containing antacids
- Caution with drugs metabolized through CYP3A4
- Consider dose increase with concomitant use of specific drugs
- Caution with drugs with ulcerogenic potential or anticoagulants
Contraindications
- Hypersensitivity to the active substance or excipients
- Creatinine clearance less than 40 ml/min or serum creatinine >2 times the upper limit of normal
Side Effects
- Cytopenias may occur
- Gastrointestinal effects like diarrhea, constipation, etc.
- Elevation of hepatic transaminases
- Rare cases of Stevens-Johnson syndrome and skin rash
- Auditory and ocular disturbances may occur
Pregnancy & Lactation
- Category C drug
- No adequate and well-controlled studies in pregnant women
- Potential risk to the fetus
Precautions & Warnings
- Fatal and non-fatal gastrointestinal bleeding and irritation may occur
- Monitor blood counts during therapy
- Serious hypersensitivity reactions reported
- Use caution in combination with drugs having ulcerogenic or hemorrhagic potential
Use in Special Populations
- Safety and efficacy in pediatric patients similar to adult patients
- Similar response in younger and older pediatric patients
- Elderly patients at increased risk for toxicity
- Not studied in patients with renal or hepatic impairment
