Indications

  • Treatment of acute clinical manifestations of Herpes simplex virus in immunocompromised patients
  • Treatment of severe primary or non-primary genital herpes in immune competent patients
  • Treatment of Varicella zoster virus infection in immunocompromised patients
  • Treatment of Herpes zoster (shingles) in immune competent patients with severe acute local or systemic manifestations
  • Treatment of Herpes simplex encephalitis

Pharmacology

  • Interferes with DNA synthesis
  • Inhibits viral replication
  • Antiviral activity against Herpes simplex virus
  • Antiviral activity against Varicella zoster virus
  • Requires intracellular conversion to Acyclovir Triphosphate for activity

Dosage

  • 5 mg/kg every 8 hours for Herpes simplex infection
  • 5 mg/kg every 8 hours for severe Herpes zoster infection
  • 10 mg/kg every 8 hours for Varicella zoster infection
  • 10 mg/kg every 8 hours for Herpes simplex encephalitis
  • Each dose administered by slow intravenous infusion over a one-hour period

Administration

  • Recommended duration of administration: 5 to 7 days for most infections
  • Recommended duration of administration: at least 10 days for Herpes simplex encephalitis
  • Administered via controlled-rate infusion pump or further diluted for administration by infusion
  • Compatible with various infusion fluids
  • Reconstituted and diluted solution to be used immediately and any unused solution to be discarded

Interaction

  • Co-administration of probenecid increases mean half-life and area under the concentration time curve
  • Concurrent use of diuretics increases plasma levels in patients over 60 years of age

Contraindications

  • Hypersensitivity to Acyclovir or Valacyclovir

Side Effects

  • Lethargy
  • Confusion
  • Reversible increases in liver-related enzymes
  • Nausea
  • Local inflammatory reactions if inadvertently infused into extracellular tissues

Pregnancy & Lactation

  • Pregnancy category B
  • Not recommended during pregnancy unless benefits clearly outweigh potential risks to fetus
  • Administered to nursing mothers only if benefits to mother outweigh potential risks to the baby

Precautions & Warnings

  • Intended for intravenous infusion only
  • Reconstituted solution should not be administered by mouth
  • Must be given over a period of at least one hour to avoid renal tubular damage
  • Must be accompanied by adequate hydration

Use in Special Populations

  • Pediatric use: dose calculated based on body surface area
  • Pediatric dose for Herpes simplex and Varicella zoster infections: 250 mg/m2
  • Geriatric use: special attention needed due to often low creatinine clearance
  • Patients with renal impairment: dosage modifications recommended based on creatinine clearance

Overdose Effects

  • Elevations of serum creatinine
  • Neurological effects including confusion, seizures, and coma
  • Adequate hydration essential to reduce possibility of crystal formation in urine

Reconstitution

  • Each 250 mg vial reconstituted with 10 ml of Water for Injection or Sodium Chloride Intravenous Infusion (0.9% w/v)
  • Each 500 mg vial reconstituted with 10 ml of Water for Injection or Sodium Chloride Intravenous Infusion (0.9% w/v)
