Pharmacology
Mecobalamin is a coenzyme form of vitamin B12. It is a cofactor in the enzyme methionine synthase, required for myelin integrity, neuronal function, RBC formation, and DNA synthesis. It helps in the synthesis of neuronal lipids, axonal nerves regeneration, promotes axonal transport, and has neuroprotective activity.
Dosage & Administration
- Tablet: 0.5 mg tablet 3 times daily after meal. Dosage adjusted according to age and symptoms severity.
- Injection:
- 1 ampoule (Mecobalamin 500 mcg) daily, IM or IV 3 times a week for peripheral neuropathies in adults
- 1 ampoule (Mecobalamin 500 mcg) daily, IM or IV 3 times a week for megaloblastic anemia in adults. After 2 months, reduce to a single administration at intervals for maintenance therapy.
- Use in Children: Not Recommended
Interaction
Absorption of vitamin B12 from the gastrointestinal tract may be reduced by certain medications.
Contraindications
Hypersensitivity to Mecobalamin or other components of the formulation.
Side Effects
- Dermatologic Effects: Rash
- Gastrointestinal Effects: Anorexia, nausea/vomiting, diarrhea
- Neurologic Effects (CNS): Headache
- Others:
- Anaphylactoid reaction: decrease in blood pressure or dyspnea
- Hot sensation
- Diaphoresis
- Pain/induration at the site of intramuscular injection

Precautions & Warnings
Mecomin should not be used aimlessly for more than one month unless effective. Prolonged use of larger doses not recommended for patients handling mercury or mercury compounds.
