Indications
- Treatment of folison deficiency
- Treatment of megaloblastic anemia
- Treatment of anemias of nutritional origins, pregnancy, infancy, or childhood
- Maintenance of normal erythropoiesis
- Synthesis of purine and thymidylate nucleic acids
Pharmacology
- Conversion of folic acid to tetrahydrofolic acid and methyltetrahydrofolate by DHFR
- Transportation of folic acid congeners across cells by receptor-mediated endocytosis
- Maintenance of normal erythropoiesis
- Synthesis of purine and thymidylate nucleic acids
- Interconversion of amino acids
- Methylation of tRNA
- Generation and use of formate
- Normalization of high homocysteine levels by remethylation of homocysteine to methionine via vitamin B12
Dosage & Administration
- Initial adult dose of 5 mg daily for 4 months
- Maintenance adult dose of 5 mg every 1-7 days depending on the underlying disease
- Children dose: up to 1 year - 500 mcg/kg daily, over 1 year - same as adult dose

- Generally well tolerated
- Gastro-intestinal disturbances may occur
- Rare reports of hypersensitivity reactions

No special precautions known
Precautions & Warnings
- Use with caution in patients who may have folate dependent tumours
- Should never be given alone or in conjunction with inadequate amounts of vitamin B12 for the treatment of undiagnosed megaloblastic anaemia
- Should not be given alone in vitamin B12 deficiency states as it may precipitate the onset of subacute combined degeneration of the cord
- Cobalamin absorption test should be done in elderly people before long-term folate
- No harmful results found from short courses of folate
