Indications
- Folus deficiency
- Megaloblastic anemia
- Anemias of nutritional origins, pregnancy, infancy, or childhood
Pharmacology
- Conversion to tetrahydrofolic acid and methyltetrahydrofolate by dihydrofolate reductase (DHFR)
- Transport across cells by receptor-mediated endocytosis
- Role in maintaining normal erythropoiesis, synthesizing purine and thymidylate nucleic acids, interconverting amino acids, methylating tRNA, and generating and using formate
- Normalization of high homocysteine levels by remethylation of homocysteine to methionine via methionine synthetase with the help of vitamin B12 as a cofactor
Dosage & Administration
- Adults:
- Initially: 5 mg daily for 4 months
- Maintenance: 5 mg every 1-7 days depending on the underlying disease
- Children:
- Up to 1 year: 500 mcg/kg daily
- Over 1 year: Same as adult dose

- Previous intolerance to folic acid

- Gastro-intestinal disturbances may occur
- Rare hypersensitivity reactions reported

- No special precautions known
Precautions & Warnings
- Caution in patients with folate dependent tumors
- Avoid use alone or in inadequate amounts of vitamin B12 for undiagnosed megaloblastic anemia
- Not to be given alone in vitamin B12 deficiency states to prevent subacute combined degeneration of the cord
- Cobalamin absorption test recommended in elderly before long term folate use
- No harmful results found from short courses of folate
