Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients on dialysis
- Treatment of anemia in Chronic Kidney Disease (CKD) patients not on dialysis
- Treatment of anemia in HIV-infected patients on Zidovudine
- Reduction of allogeneic RBC transfusions in noncardiac, nonvascular surgery patients
- Prevention of anemia of prematurity in infants with low birth weight and gestational age
Description
- Recombinant human erythropoietin (EPO) with 165 amino acid sequence
- Expressed in Chinese hamster ovary cells
- Apparent molecular weight of erythropoietin is about 30,400 daltons
Dosage
- Evaluation of iron stores and nutritional factors before and during treatment
- Monitoring hemoglobin levels at least weekly when initiating or adjusting therapy
- Dosage adjustments based on hemoglobin rates and responsiveness
- Different starting doses for CKD patients on dialysis and not on dialysis
- Starting dose for Zidovudine-treated HIV-infected patients
Administration
- Do not shake or freeze Erythropoietin alfa
- Protect syringe from light
- Visual inspection for particulate matter and discoloration before administration
- Discard unused portions of Erythropoietin alfa
Interaction
- Potentiation of Epoetin effect by simultaneous therapeutic administration of a haematinic agent
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Patients who develop Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions in CKD patients, Zidovudine-treated HIV-infected patients, cancer patients on chemotherapy, surgery patients, and premature infants
Pregnancy & Lactation
- Use in pregnancy only if potential benefit outweighs potential risk to the fetus
- Not recommended in pregnant or lactating surgical patients participating in autologous blood predonation program
- Caution in nursing women, decision based on benefit of breast feeding and Erythropoietin alfa therapy
Overdose Effects
- Hemoglobin levels above desired level
- Discontinuation or reduction of Epoetin dosage and/or phlebotomy management
- Cases of severe hypertension observed following overdose with ESAs
