Indications
- Treatment of anemia due to chronic kidney disease (CKD) in patients on dialysis and not on dialysis
- Treatment of anemia due to Zidovudine in HIV-infected patients
- Treatment of anemia due to the effects of concomitant myelosuppressive chemotherapy
- Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with a birth weight of 750 to 1500 gm and gestational age of less than 34 weeks
Description
- Recombinant human erythropoietin (EPO)
- Expressed in Chinese hamster ovary cells
- Has a 165 amino acid sequence identical to that of human urinary EPO
- Molecular weight of erythropoietin is about 30,400 daltons
Dosage
- Evaluation of iron stores and nutritional factors before and during treatment
- Monitor hemoglobin levels at least weekly until stable, then at least monthly
- Do not increase the dose more frequently than once every 4 weeks
- For patients who do not respond adequately, increase the dose by 25%
Administration
- Do not shake or freeze Erythropoietin alfa
- Protect syringe from light
- Inspect visually for particulate matter and discoloration prior to administration
Interaction
- Effect may be potentiated by the simultaneous therapeutic administration of a haematinic agent such as ferrous sulphate
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Patients with Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions in patients with CKD: Hypertension, arthralgia, muscle spasm, pyrexia, dizziness, medical device malfunction, vascular occlusion, upper respiratory tract infection
- Adverse reactions in Zidovudine-treated HIV-infected Patients: Pyrexia, cough, rash, injection site irritation
- Adverse reactions in Cancer Patients on Chemotherapy: Nausea, vomiting, myalgia, arthralgia, stomatitis, cough, weight decrease, leukopenia
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Reproduction toxicology observed in animal studies
Overdose Effects
- Eposis overdosage can cause hemoglobin levels above the desired level
- Cases of severe hypertension have been observed following overdose with ESAs
