Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients
- Treatment of anemia in patients with HIV infection receiving Zidovudine
- Reduction of allogeneic RBC transfusions in patients undergoing noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with specific birth weight and gestational age
Description
- Recombinant human erythropoietin (EPO)
- Expressed in Chinese hamster ovary cells
- 165 amino acid sequence identical to human urinary EPO
- Apparent molecular weight of erythropoietin is about 30,400 daltons
Dosage
- Evaluation of iron stores and nutritional factors before and during treatment
- Monitoring hemoglobin levels at least weekly until stable, then at least monthly
- Dose adjustment considerations for patients with CKD on dialysis
- Dose adjustment considerations for patients with CKD not on dialysis
- Starting dose for Zidovudine-treated HIV-infected patients
- Dose adjustment considerations for patients on cancer chemotherapy
- Dosing regimen for prevention of anemia of prematurity
Administration
- Do not shake the product
- Protect syringe from light
- Visual inspection for particulate matter and discoloration prior to administration
- Discard unused portions of Erythropoietin alfa
Interaction
- Potentiation by simultaneous therapeutic administration of haematinic agent such as ferrous sulphate
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions in different patient groups
- E.g., hypertension, arthralgia, muscle spasm, pyrexia, dizziness, medical device malfunction, vascular occlusion, upper respiratory tract infection
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Reproduction toxicology in animal studies
- Caution in use during pregnancy and lactation
- Excretion in human milk is not known
Overdose Effects
- Causes hemoglobin levels above the desired level
- Management with discontinuation or reduction of Eposis dosage and/or with phlebotomy
- Cases of severe hypertension observed following overdose with ESAs
