Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients
- Treatment of anemia in Zidovudine-treated HIV-infected patients
- Treatment of anemia in patients undergoing myelosuppressive chemotherapy
- Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with specific birth weight and gestational age
Description
- Recombinant human erythropoietin (EPO)
- Expressed in Chinese hamster ovary cells
- Has a 165 amino acid sequence identical to that of human urinary EPO
- Apparent molecular weight of about 30,400 daltons
Storage Conditions
- Store at 2°C to 8°C
- Do not freeze or shake
- Store in original package to protect from light
Dosage
- Evaluate iron status and maintain iron repletion before and during treatment
- Monitor hemoglobin levels at least weekly until stable, then at least monthly
- Consider initiating treatment based on specific hemoglobin levels
- Do not increase the dose more frequently than once every 4 weeks
- Adjust dose based on hemoglobin response
Administration
- Do not shake or use if shaken or frozen
- Protect syringe from light
- Inspect visually for particulate matter and discoloration before administration
- Discard unused portions
Interaction
- Effect may be potentiated by simultaneous therapeutic administration of haematinic agents
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- History of Pure Red Cell Aplasia (PRCA) following erythropoietin treatment
Side Effects
- Adverse reactions in CKD patients, Zidovudine-treated HIV-infected Patients, Cancer Patients on Chemotherapy, Surgery Patients, and Premature infants
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Reproduction toxicology observed in animal studies
- Use in pregnancy only if potential benefit outweighs potential risk to the fetus
- Not recommended in pregnant or lactating surgical patients participating in autologous blood predonation program
- Caution in nursing women with decision based on benefit of breast feeding and Erythropoietin alfa therapy
Overdose Effects
- Nipotin overdosage may cause hemoglobin levels above desired level
- Manage with discontinuation or reduction of dosage and/or phlebotomy
- Severe hypertension observed following overdose with ESAs
