Indications
- Post-menopausal osteoporosis
- Renal osteodystrophy
- Postsurgical hypoparathyroidism

- Secondary hyperparathyroidism in patients with moderate to severe chronic renal failure (pre-dialysis)

Pharmacology
- Active metabolite of vitamin D3
- Promotes intestinal absorption of calcium
- Regulates bone mineralization
- Stimulates osteoblastic activity in the skeleton
Dosage & Administration
- Optimal daily dose based on serum calcium level
- Recommended dose for Post-menopausal Osteoporosis: 0.25 mcg twice daily
- Initial dose for Renal Osteodystrophy: 0.25 mcg daily
- Initial dosage for Hypoparathyroidism and Rickets: 0.25 mcg/day
- Intravenous initial dose for hypocalcemia and/or secondary hyperparathyroidism: 1-2 mcg administered three times weekly
- Dosage adjustment for pediatric patients
Interaction
- Increased risk of hypercalcemia with thiazide diuretics
- Careful dosage determination for patients on digitalis
- Potential hypermagnesemia with magnesium-containing drugs
Contraindications
- Hypersensitivity to any of its ingredients
- Diseases associated with hypercalcemia
Side Effects
- Early side effects: weakness, headache, nausea, constipation, muscle pain
- Late side effects: polyuria, anorexia, weight loss, conjunctivitis, hyperthermia, hypertension, ectopic calcification
Pregnancy & Lactation
- Use during pregnancy if benefits outweigh potential risk
- Monitoring of serum calcium levels for breastfeeding mothers and infants
Precautions & Warnings
- Excessive dosage induces hypercalcemia and hypercalciuria
- Cautious use in patients on digitalis
- Appropriate titration downward upon discontinuation
- Withholding of vitamin D and its derivatives during treatment
- Monitoring of serum phosphorus levels in dialysis patients
