Indications
- Post-menopausal osteoporosis
- Renal osteodystrophy
- Postsurgical hypoparathyroidism
- Idiopathic hypoparathyroidism
- Pseudohypoparathyroidism
- Secondary hyperparathyroidism in patients with chronic renal failure
- Vitamin D dependent rickets
Pharmacology
Calcitriol promotes intestinal absorption of calcium and regulates bone mineralization.
Dosage & Administration
- Recommended dose for Post-menopausal Osteoporosis: 0.25 mcg twice daily
- Recommended dose for Renal Osteodystrophy: 0.25 mcg initially, adjust as per calcium levels
- Recommended dose for Hypoparathyroidism and Rickets: 0.25 mcg/day, may increase if needed
Interactions
Thiazide diuretics increase risk of hypercalcemia, caution with digitalis treatment
Contraindications
Hypersensitivity to ingredients, diseases associated with hypercalcemia
Side Effects
- Early side effects: weakness, headache, nausea, muscle pain
- Late side effects: polyuria, anorexia, photophobia, hypertension
Pregnancy & Lactation
May be used during pregnancy if benefits outweigh risk, monitor serum calcium levels
Precautions & Warnings
- Excessive dosage may induce hypercalcemia
- Discontinuation may result in rebound effect, titrate downward
- Use cautiously in patients on digitalis
Use in Special Populations
- Secondary hyperparathyroidism in Chronic Renal Failure:
- Adults and pediatric patients 3 years or older: 0.25 mcg/day, may increase to 0.5 mcg/day if necessary
- Pediatric patients less than 3 years old: 10 to 15 ng/kg/day
- Elderly: No dosage adjustment required
Overdose Effects
Can cause hypercalcemia, hypercalciuria and hyperphosphatemia
