Indications
- Post-menopausal osteoporosis
- Renal osteodystrophy
- Postsurgical hypoparathyroidism
- Idiopathic hypoparathyroidism
- Pseudohypoparathyroidism
- Secondary hyperparathyroidism in patients with moderate to severe chronic renal failure (pre-dialysis)
- Vitamin D dependent rickets
Pharmacology
- Calcitriol as active metabolite of vitamin D3
- Promotes intestinal absorption of calcium
- Regulates bone mineralization
- Stimulates osteoblastic activity in the skeleton

Interaction
- Concomitant treatment with thiazide diuretics increases risk of hypercalcemia
- Careful dosage determination in patients undergoing digitalis treatment
- Magnesium containing drugs may cause hypermagnesemia
Contraindications
- Hypersensitivity to any ingredients
- Diseases associated with hypercalcemia
Side Effects
- Early side effects include weakness, headache, nausea, vomiting
- Late side effects include polyuria, anorexia, weight loss
- Rare side effects include overt psychosis
Pregnancy & Lactation
- No evidence of teratogenicity in humans
- Use during pregnancy if benefits outweigh potential risk to fetus
- Breastfeeding possible but with monitored serum calcium levels
Precautions & Warnings
- Excessive dosage may induce hypercalcemia and hypercalciuria
- Cautious use in patients on digitalis
- Appropriate titration downward for maintenance dose
- Discontinuation may result in rebound effect
- Chronic hypercalcemia can lead to vascular calcification, nephrocalcinosis
Use in Special Populations
- Different dosage for adults, pediatric patients, and elderly
- Specific dosage for pediatric patients less than 3 years of age
Overdose Effects
- Hypercalcemia, hypercalciuria, hyperphosphatemia
- Immediate discontinuation of therapy, low calcium diet
