Indications

• Prevention and treatment of secondary hyperparathyroidism associated with Chronic Kidney Disease (CKD) Stages 3 and 4
• Prevention and treatment of secondary hyperparathyroidism associated with CKD Stage 5 in patients on hemodialysis (HD) or peritoneal dialysis (PD)

Pharmacology

• Synthetic, biologically active vitamin D2 analog
• Mediated through binding of the VDR
• Selective activation of vitamin D responsive pathways
• Reduction of parathyroid hormone levels by inhibiting PTH synthesis and secretion

Dosage & Administration

• Can be taken without regard to food
• Different dosing schedules for CKD Stages 3 and 4 vs Stage 5
• Recommended starting dose based upon baseline intact parathyroid hormone (IPTH) level
• Dose titration based upon IPTH level relative to baseline
• Monitoring of serum calcium and phosphorus levels

Interaction

• Effects of strong CYP3A inhibitors on exposure of the product

Contraindications

• Should not be given to patients with evidence of hypercalcemia or vitamin D toxicity

• Adverse reaction rates observed in clinical studies

Pregnancy & Lactation

• Limited data on use during pregnancy
• Risks associated with chronic kidney disease in pregnancy

Overdose Effects

• Excessive administration causing hypercalcemia, hypercalciuria, hyperphosphatemia, and over suppression of PTH
