Indications
- Treatment of thiamine deficiency
- Beriberi
- Wernick's encephalopathy
- Neuritis associated with pregnancy
- Pellagra
- Prophylactic use in low dietary intake
- Prophylactic use in impaired gastrointestinal absorption of thiamine
- Prophylactic use in increased requirements (e.g. pregnancy, carbohydrate-rich diet)
Pharmacology
- Coenzyme for decarboxylation of α-ketoglutaric acid
- Necessary for optimal growth of infants and children
- Affects peripheral nervous system, gastrointestinal tract, and cardiovascular system
- Not stored in the body
- Lost from tissues during deficiency
- Required daily
- Deficiency leads to fatigue, anorexia, gastrointestinal disturbance, tachycardia, irritability, and neurological symptoms
- Well absorbed from the gastrointestinal tract
- Rapid absorption from the upper small intestine
- Not stored in the body to any appreciable extent
- Excess ingested thiamine appears in urine
- Plasma half-life of 24 hours
Dosage & Administration
- Prophylaxis: 3 to 10 mg daily
- Mild chronic deficiency: 10 to 25 mg daily
- Severe deficiency: 200 to 300 mg daily

- No hazardous drug interactions reported
- Acts synergistically with other B-complex vitamins
- Reduced potential for causing adverse effects
Contraindications
- Increased risk of anaphylaxis with repeated parenteral administration
- Warning signs of mild allergic phenomena
- Avoid intravenous administration except in comatose patients
- No need for parenteral administration once deficiency is corrected
Side Effects

- Anaphylactic reactions after intravenous or intramuscular administration in sensitive patients
- Risk of reactions increases with repeated administration
