Indications

  • Treatment of thiamine deficiency
  • Beriberi
  • Wernick's encephalopathy
  • Neuritis associated with pregnancy
  • Pellagra
  • Prophylactic use in low dietary intake
  • Prophylactic use in impaired gastrointestinal absorption of thiamine
  • Prophylactic use in increased requirements (e.g. pregnancy, carbohydrate-rich diet)

Pharmacology

  • Coenzyme for decarboxylation of α-ketoglutaric acid
  • Necessary for optimal growth of infants and children
  • Affects peripheral nervous system, gastrointestinal tract, and cardiovascular system
  • Not stored in the body
  • Lost from tissues during deficiency
  • Required daily
  • Deficiency leads to fatigue, anorexia, gastrointestinal disturbance, tachycardia, irritability, and neurological symptoms
  • Well absorbed from the gastrointestinal tract
  • Rapid absorption from the upper small intestine
  • Not stored in the body to any appreciable extent

Dosage & Administration

  • Prophylaxis: 3 to 10 mg daily
  • Mild chronic deficiency: 10 to 25 mg daily
  • Severe deficiency: 200 to 300 mg daily

  • Acts synergistically with other B-complex vitamins
  • Reduced potential for causing adverse effects

Contraindications

  • Increased risk of anaphylaxis with repeated parenteral administration
  • Warning signs of mild allergic phenomena
  • Avoid intravenous administration except in comatose patients
  • No need for parenteral administration once deficiency is corrected

  • Anaphylactic reactions after intravenous or intramuscular administration in sensitive patients
