Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Prostaglandin synthesis inhibition
  • Proton pump inhibitor
  • Gastric acid secretion suppression

Dosage

  • Use lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole twice daily
  • Adolescent dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole based on weight

Administration

  • Take at least 30 minutes before meals

Interaction

  • Concomitant use with NSAIDs may reduce antihypertensive effect
  • Concomitant use with warfarin may increase risk of bleeding
  • Inhibition of gastric acid secretion
  • Interference with drug absorption

Contraindications

  • Hypersensitivity to any component
  • History of allergic reactions
  • Use during peri-operative period in CABG surgery

  • Lower risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension, cardiac failure

Pregnancy & Lactation

  • Not recommended during first and second trimester
  • Contraindicated during third trimester
  • Not recommended during breastfeeding
  • May impair female fertility

  • Avoid use with NSAIDs including COX-2 selective inhibitors
  • Minimize undesirable effects with lowest effective dose
  • Regular surveillance for long-term treatment
  • Increased frequency of adverse reactions in older people
  • Gastrointestinal, cardiovascular, cerebrovascular, and renal effects

Use in Special Populations

  • Elderly patients caution
  • Not recommended for moderate to severe renal impairment
  • Close monitoring for mild to moderate hepatic impairment
