Indications
- 250 mg or 500 mg tablet
- 1000 mg tablet
- Treatment or prevention of calcium depletion
- Conditions associated with calcium deficiency
- Management of conditions associated with hyperacidity
- Fast relief of acid indigestion, heartburn, sour stomach, upset stomach
Pharmacology
- Reacts with gastric acid to produce a salt and water
- Chemical reaction: CaCO3+2HCl = CaCl2+H2O+CO2
- Increases gastric pH, diminishes pepsin activity
- Up to 30% oral calcium load absorption
Dosage & Administration
- Recommended oral use
- Adult doses equivalent to 540-2000 mg Calcium Carbonate per day
- Children's doses half of those for adults
- Pregnancy and lactation doses 1200-1500 mg per day
- Chronic renal failure doses vary from 2.5-9.0 gm per day
- Maximize effective phosphate binding by giving with meals
- For 1000 mg tablet: 2000-3000 mg tablet when symptoms occur; may be repeated hourly if needed or as directed by the physician
Interaction
- Enhances cardiac effects of digoxin and other cardiac glycosides
- Interferes with absorption of concomitantly administered tetracycline preparations
- Requires modification of vitamin D therapy in chronic renal failure
Contraindications
- Hypercalcaemia and hyperparathyroidism
- Hypercalciuria and nephrolithiasis
- Zollinger-Ellison syndrome
- Concomitant digoxin therapy (requires careful monitoring of serum calcium level)
- Use cautiously in patients with sarcoidosis, renal or cardiac disease, and in patients receiving cardiac glycosides
Side Effects
- May be irritating to the GI tract
- May cause constipation
- Rarely produces hypercalcaemia
Pregnancy & Lactation
- Widely used in pregnancy for oral calcium supplementation or antacid therapy
- Can be used in lactating women
Use in Special Populations
- Safe and effective use in children with chronic renal failure
- Constipation may be troublesome for elderly patients with renal failure
