Dosage & Administration
- Orally administered
- Recommended doses for adults: 540-2000 mg Calcium Carbonate per day
- Recommended doses for children: Half of those for adults
- Recommended daily dose of calcium in pregnancy and lactation: 1200-1500 mg
- Doses in chronic renal failure: 2.5-9.0 gm Calcium Carbonate per day
Indications
- Treatment or prevention of calcium depletion
- Conditions associated with calcium deficiency: hypoparathyroidism, achlorhydria, chronic diarrhea, vitamin D deficiency, steatorrhea, sprue, pregnancy and lactation, menopause, pancreatitis, renal failure, alkalosis, hyperphosphataemia
Pharmacology
- React with gastric acid to produce a salt and water
- Increase in gastric pH diminishes the activity of pepsin in the gastric secretion
- Up to 30% of the oral calcium load may be absorbed
Interaction
- May enhance the cardiac effects of digoxin and other cardiac glycosides if systemic hypercalcaemia occurs
- Interfere with the absorption of concomitantly administered tetracycline preparations
- Modification of vitamin D therapy may be required in chronic renal failure to avoid hypercalcaemia
Contraindications
- Hypercalcaemia and hyperparathyroidism
- Hypercalciuria and nephrolithiasis
- Zollinger-Ellison syndrome
- Concomitant digoxin therapy (requires careful monitoring of serum calcium level)
- Use cautiously in patients with sarcoidosis, renal or cardiac disease, and in patients receiving cardiac glycosides
Side Effects
- Irritating to the GI tract when orally administered
- May cause constipation
- Rarely produces hypercalcaemia, especially in large doses to patients with chronic renal failure
Pregnancy & Lactation
- Can be used in pregnancy by way of oral calcium supplementation or antacid therapy
- Can be used in lactating women
Use in Special Populations
- Safe and effective in children and infants with chronic renal failure
- In elderly patients with renal failure, constipation may be troublesome
