Indications
- Used to treat carbohydrate and fluid depletions
- Replacement for water deficiency
- Treatment for coma, dysphagia, and dehydration in elderly individuals
- Provides readily metabolizable nutrient
- Used in regimens with calcium bicarbonate and insulin for emergency management of hyperkalemia
- Suitable vehicle for slow intravenous infusion of drugs
Pharmacology
- Form of glucose (sugar)
- Injected into a vein through an IV to replace lost fluids
- Provides carbohydrates to the body
- Used for low blood sugar, insulin shock, and dehydration
Dosage
- Dose is variable and dependent on individual patient requirements
- Maximum rate of infusion without causing glycosuria is 0.5 g/kg/hour for 5% dextrose solution
- Restoration of normal circulation by infusing 20-30 ml/kg as rapidly as possible for 10% dextrose solution
- Maximum rate of dextrose infusion for 25% solution without producing glycosuria is 0.5 g/kg/hr
- Recommended flow rate for adults is 10-35 drops per minute for 25% dextrose solution
- Oral powder dosage for adults and children
Administration
- Inspection of infusion set and solution prior to use
- Procedure for inserting the spike of the administration set to the Eurocap
- Cleaning and securing the venipuncture site
- Infusion set-up and gradually adjusting drip speed
Contraindications
- Allergy to dextrose
- Existence of diabetes, breathing problems, electrolyte imbalance, kidney or liver disease, food or drug allergy, or regular blood transfusions
Side Effects
- Severe burning, pain, or swelling around the IV needle
- Warmth, redness, oozing, or bleeding at the IV insertion site
- Symptoms of high blood sugar, headache, trouble concentrating, weakness, hallucinations, fainting, seizure, shallow or stopped breathing
- Low potassium symptoms like anxiety, sweating, pale skin, shortness of breath, wheezing, pain, fast or uneven heart rate
Pregnancy & Lactation
- FDA pregnancy category C
- Unknown effects on unborn baby
- Concerns about passing into breast milk
Precautions & Warnings
- Careful monitoring of serum glucose concentration
- Possible concurrent use of insulin for diabetic patients
- Avoid mixing with whole blood to prevent haemolysis and clumping
- Immediate discontinuation of infusion if rigor arises
- Avoid use if solution is cloudy, contains particles, or after expiry date
- Check with doctor before usage if allergic to the product
