Indications

  • Treatment of gestational choriocarcinoma, chorioadenoma destruens and hydatidiform mole
  • Management of selected adults with severe, active rheumatoid arthritis
  • Symptomatic control of severe, recalcitrant, disabling psoriasis

Pharmacology

  • Inhibits dihydrofolic acid reductase
  • Interferes with DNA synthesis and cellular replication
  • More sensitive to effect in actively proliferating tissues

Dosage

  • Orally or intramuscularly in doses of 15 to 30 mg daily for a five-day course for choriocarcinoma
  • Induction dose of 3.3 mg/m2 in acute lymphoblastic leukemia
  • Single doses of 7.5 mg once per week for adult rheumatoid arthritis

Administration

Best taken on an empty stomach. May be taken with meals to reduce GI discomfort. Avoid taking with milk-rich products

Interaction

  • NSAIDs should not be administered prior to or concomitantly with high doses
  • Toxicity may be increased with certain medications
  • May decrease intestinal absorption of Methotrexate

Contraindications

  • Alcoholism or chronic liver disease
  • Immunodeficiency syndromes
  • Hypersensitivity to methotrexate
  • Pregnancy and lactation

Side Effects

  • Ulcerative stomatitis
  • Leukopenia
  • Nausea and abdominal distress

US FDA Pregnancy Category X. Contraindicated in nursing mothers

Precautions & Warnings

  • Methotrax formulations and diluents containing preservatives must not be used for intrathecal or high dose therapy
  • Necessary to follow patients closely for toxic effects
  • If adverse reactions occur, the drug should be reduced in dosage or discontinued

Use in Special Populations

Benefits should be weighed against the potential risk before using in pediatric or geriatric patients

Overdose Effects

Leucovorin is indicated to counteract the effect of overdosages
