Indications

  • Treatment of gestational choriocarcinoma, chorioadenoma destruens and hydatidiform mole

Pharmacology

Dosage

  • Orally or intramuscularly in daily doses of 15 to 30 mg for a five-day course
  • Induction and maintenance doses for acute lymphoblastic leukemia
  • Different dosing for various stages of lymphoma
  • Intravenous dosage for breast cancer and head and neck cancer
  • Different single doses for adult rheumatoid arthritis, polyarticular-course juvenile rheumatoid arthritis, and psoriasis

Administration

Should be taken on an empty stomach, may be taken with meals to reduce GI discomfort, avoid taking with milk-rich products

Interaction

  • NSAIDs should not be administered prior to or concomitantly with high doses of methotrexate
  • Toxicity may increase with salicylates, phenylbutazone, phenytoin, and sulfonamides
  • Should be closely monitored for possible increased risk of hepatotoxicity when combined with sulfasalazine
  • Caution must be exercised if high-dose methotrexate is administered in combination with cisplatin
  • Increases the plasma levels of mercaptopurine
  • May decrease intestinal absorption with Tetracycline, Chloramphenicol, and Nonabsorbable Broad Spectrum Antibiotics
  • Decreases responses to systemically administered methotrexate with Vitamin preparations containing folic acid or its derivatives

Contraindications

  • Alcoholism, alcoholic liver disease, or other chronic liver disease
  • Immunodeficiency syndromes
  • Pre-existing blood dyscrasias
  • Hypersensitivity to methotrexate
  • Pregnant women with psoriasis or rheumatoid arthritis
  • Women of childbearing potential
  • Nursing mothers

  • Ulcerative stomatitis
  • Leukopenia
  • Nausea and abdominal distress
  • Malaise
  • Fatigue
  • Chills and fever
  • Dizziness
  • Decreased resistance to infection
  • Impairment of fertility, oligospermia, and menstrual dysfunction

Pregnancy & Lactation

US FDA Pregnancy Category X, contraindicated in nursing mothers

Precautions & Warnings

  • Formulations and diluents containing preservatives must not be used for intrathecal or high dose methotrexate therapy
  • Necessary to follow patients on methotrexate closely for toxic effects
  • Adverse reactions may necessitate dosage reduction or discontinuation
  • Persistent liver function test abnormalities require evaluation
  • Folate deficiency may increase methotrexate toxicity

Use in Special Populations

  • Benefits should be weighed against the potential risk before use in pediatric patients
  • Elderly patients should be closely monitored for early signs of hepatic, bone marrow, and renal toxicity

Overdose Effects

Leucovorin is indicated to diminish the toxicity and counteract the effect of inadvertently administered overdosages of methotrexate
