Indications

Pharmacology
Inhibits dihydrofolic acid reductase, interferes with DNA synthesis, repair, and cellular replication
Dosage
Doses for various neoplastic diseases, leukemia, lymphoma, mycosis fungoides, breast cancer, head and neck cancer, rheumatoid arthritis, juvenile rheumatoid arthritis, and psoriasis
Administration
Should be taken on an empty stomach, may be taken with meals to reduce GI discomfort, avoid taking with milk-rich products
Interaction
- Interaction with NSAIDs, salicylates, phenylbutazone, phenytoin, sulfonamides, cisplatin, mercaptopurine, tetracycline, chloramphenicol, nonabsorbable broad spectrum antibiotics, vitamin preparations containing folic acid or its derivatives, trimethoprim/sulfamethoxazole
Contraindications
- Alcoholism or alcoholic liver disease
- Chronic liver disease
- Immunodeficiency syndromes
- Pre-existing blood dyscrasias
- Hypersensitivity to methotrexate
- Pregnant women with psoriasis or rheumatoid arthritis
- Women of childbearing potential
- Nursing mothers

Precautions & Warnings
- Formulations and diluents containing preservatives must not be used for intrathecal or high dose therapy
- Necessary to closely follow patients for toxic effects
- If adverse reactions occur, reduce dosage or discontinue
- Consideration for the use of leucovorin calcium if necessary
- Increased alertness as to possible recurrence of toxicity
- Evaluation of persistent liver function test abnormalities
- Consideration of possible folate deficiency
Use in Special Populations
- Pediatric patients: Benefits vs. potential risk
- Geriatric patients: Monitor for signs of hepatic, bone marrow, and renal toxicity
Overdose Effects
Leucovorin is indicated to diminish the toxicity and counteract the effect of inadvertently administered overdosages
