Pharmacology

  • Dopamine antagonist

Dosage and Administration

  • Taken 15-30 minutes before meals
  • Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily
  • Usual recommended oral dose for children: 2-4 ml suspension/10 kg body weight every 6-8 hours daily

Interaction

  • Anticholinergic drugs may antagonize the antidyspeptic effect
  • Concomitant use of drugs that significantly inhibit CYP3A4 may result in increased plasma levels of domperidone

Contraindications

  • Known hypersensitivity to the drug
  • Neonates
  • Gastrointestinal hemorrhage
  • Mechanical obstruction or perforation
  • Prolactin releasing pituitary tumor (prolactinoma)

Side Effects

  • Transient intestinal cramps
  • Extrapyramidal phenomena
  • Increase in plasma prolactin level
  • Rare allergic reactions such as rash and urticaria

Pregnancy and Lactation

  • Use during the first trimester if justified by therapeutic benefit
  • Excreted in breast milk
  • Concentrations in breast milk are 4 times lower than corresponding plasma concentrations
  • Not recommended for nursing mothers unless expected benefits outweigh potential risk

Precautions and Warnings

  • Absolute caution in the case of children
  • Increased risk of extrapyramidal reactions in young children
  • Use with caution in patients with hepatic impairment

Use in Special Populations

  • Caution in infants due to incomplete blood-brain barrier
