Indications
- Indicated in acute leukemia
- Useful in combination with other oncolytic agents in Hodgkin's disease
- Useful in non-Hodgkin's malignant lymphomas
- Useful in rhabdomyosarcoma
- Useful in neuroblastoma
- Useful in Wilms' tumor
Pharmacology
- Derived from periwinkle plant
- White or slightly yellow in color
- Hygroscopic
- Amorphous or crystalline powder
- Freely soluble in water
- Slightly soluble in alcohol
- Triphasic serum decay pattern
- Liver is major excretory organ
- 80% excreted in faeces
- 10-20% excreted in urine
- Rapid distribution from blood into tissue
- Tightly bound in tissue
Dosage & Administration
- Extreme care required in calculating and administering the dose
- Given intravenously at weekly intervals
- Fatal if given intrathecally
- Usual dose for adults is 1.4 mg/m2
- Usual dose for children is 1.5-2 mg/m2
- Initial dose for children weighing 10 kg or less is 0.05 mg/kg
- Administered 12 to 24 hours prior to L-asparaginase when used in combination
- Can be injected directly into a vein or tubing of an i.g. infusion
- Injection completed in about 1 minute
- Solution may be further diluted with 0.9% sodium chloride injection or 0.5% dextrose injection
Interactions
- Allopurinol may increase bone-marrow depression
- Neurotoxicity may be additive with asparaginase and isoniazid
- Concurrent use with doxorubicin and prednisone may increase myelosuppression
- Increases cellular uptake of methotrexate by malignant cells
Contraindications
- Not for patients with demyelinating form of Charcot-Marie-Tooth Syndrome
- Not to be administered to patients receiving radiation therapy through ports including the liver
- Not for patients with known hypersensitivity to vinca alkaloids or mannitol
Side Effects
- Hair loss
- Neuromuscular adverse reactions
- Hypersensitivity reactions
- Autonomic toxicity like constipation and paralytic ileus
- Genitourinary hyperuricaemia
- Hypertension and hypotension
- Neurological effects like sensory impairment, paraesthesiae, motor difficulties
- Gastrointestinal issues
- Cardiovascular effects
- Other effects such as fever, headache, defective sweating, myoclonic jerks, weight loss
Pregnancy & Lactation
- Pregnancy Category D
- Not to be used during breastfeeding
Precautions & Warnings
- Extreme care needed in administering the dose
- Needle position verification important before injection
- Leakage into surrounding tissue may cause irritation
- Vesicant properties may cause severe local reaction on extravasation
- Leucopenia less likely compared to other oncolytic agents
- Caution in patients with history of gout or urate renal stones
- Neurotoxic effects may be additive with other agents
- Acute shortness of breath and severe bronchospasm reported
- Aggressive treatment may be required for pulmonary dysfunction
