Indications
- Acute leukemia
- Hodgkin's disease
- Non-Hodgkin's malignant lymphomas

Pharmacology
- Alkaloid obtained from periwinkle plant
- Freely soluble in water
- Partially soluble in alcohol
- Triphasic serum decay pattern
- Major excretory organ - liver
- Distribution into tissue after injection
Dosage & Administration
- Extreme care required in calculating and administering dose
- Intravenous administration at weekly intervals
- Fatal if given intrathecally
- Usual dose for adults: 1.4 mg/m2
- Usual dose for children: 1.5-2 mg/m2
- Careful dosing for children weighing 10 kg or less
- Administered 12 to 24 hours prior to L-asparaginase
Interaction
- Allopurinol interaction
- Neurotoxicity with asparaginase and isoniazid
- Myelosuppression with doxorubicin and prednisone
- Cellular uptake with methotrexate
Contraindications
- Demyelinating form of Charcot-Marie-Tooth Syndrome
- Radiation therapy through ports involving the liver
- Hypersensitivity to vinca alkaloids or mannitol

Precautions & Warnings
- Proper positioning of needle before injection
- Avoid leakage into surrounding tissue
- Use by experienced physicians only
- Avoid intramuscular, subcutaneous, or intrathecal administration
- Vesicant nature causing severe local reaction
- Consideration in patients with gout or urate renal stones
- Penetrates blood-brain barrier poorly
- Neurotoxic effect additive with other agents
