Indications
- Acute leukemia
- Hodgkin's disease
- Non-Hodgkin's malignant lymphomas
- Rhabdomyosarcoma
- Neuroblastoma
- Wilms' tumor
Pharmacology
- Alkaloid obtained from periwinkle plant
- Freely soluble in water
- Slightly soluble in alcohol
- Triphasic serum decay pattern
- Major excretory organ is the liver
- 80% excreted in faeces
- 10-20% excreted in urine
- Distributed from blood into tissue within 15-30 minutes
Dosage & Administration
- Given intravenously at weekly intervals
- Fatal if given intrathecally
- Adults: 1.4 mg/m2
- Children: 1.5-2 mg/m2
- Initial dose for children weighing 10 kg or less: 0.05 mg/kg
- Solution may be injected directly into a vein or i.g. infusion tubing
- Further dilution with 0.9% sodium chloride injection or 0.5% dextrose injection if desired
Interaction
- Allopurinol may increase bone-marrow depression
- Neurotoxicity may be additive with other drugs
- Concurrent use with doxorubicin may produce increased myelosuppression
- May increase cellular uptake of methotrexate by malignant cells
Contraindications
- Demyelinating form of Charcot-Marie-Tooth Syndrome
- Do not administer while receiving radiation therapy through ports that include the liver
- Known hypersensitivity to vinca alkaloids or mannitol
Side Effects
- Reversible adverse reactions
- Most common: hair loss
- Neuromuscular adverse reactions
- Hypersensitivity reactions
- Gastrointestinal effects like constipation and paralytic ileus
- Genitourinary hyperuricaemia and uric acid nephropathy
- Cardiovascular effects like hypertension and hypotension
- Neurologic effects like sensory impairment and motor difficulties
- Other effects like fever, headache, defective sweating, and weight loss
Pregnancy & Lactation
- Pregnancy Category D
- Vincristine found in human milk
- Patients should not breastfeed
Precautions & Warnings
- Extreme care needed in calculating and administering dose
- Proper needle positioning crucial
- Extremely irritating if leaked into surrounding tissue
- Experienced physicians should handle
- Vesicant causing severe local reaction on extravasation
- Leucopenia less likely than with other oncolytic agents
- Risk/benefit consideration in patients with gout or urate renal stones
- Attention needed in patients with pre-existing neuromuscular disease or previous cytotoxic drug therapy
- Shortness of breath and severe bronchospasm reported
