Indications

Pharmacology
Cisplatin is a platinum compound that produces intra- and interstrand cross links, modifies DNA structure, inhibits DNA synthesis, and inhibits protein and RNA synthesis.
Dosage & Administration
- Metastatic Testicular Tumors: 20 mg/m2 IV daily for 5 days per cycle
- Metastatic Ovarian Tumors: 75 to 100 mg/m2 IV per cycle once every 4 weeks
- Advanced Bladder Cancer: 50 to 70 mg/m2 IV per cycle once every 3 to 4 weeks
- Non Small Cell Lung Carcinoma: 75 mg/m2 IV every three weeks
Interactions
- Additive toxicity with antineoplastic drugs
- Nephrotoxicity potentiation with aminoglycoside antibiotics
- Potential ototoxicity with aminoglycoside antibiotics or loop diuretics
- Alteration of renal elimination of bleomycin and methotrexate
- Potential decreased serum concentrations of phenytoin
- Elevated concentration of blood uric acid when concurrently receiving antigout agents
Contraindications
- Pre-existing renal impairment
- History of hypersensitivity to Cisplatin or Platinum containing compounds
- Myelosuppressed patients
- Patients with hearing impairment
Side Effects
- Severe nausea and vomiting
- Electrolyte disturbances (hypomagnesemia, hypocalcemia, hypokalemia)
- Renal tubular dysfunction
Pregnancy & Lactation
Pregnancy Category D, Cisplatin found in human milk
Precautions & Warnings
- Renal function: Produces cumulative nephrotoxicity, requires evaluation of renal function and serum electrolyte, maintenance of urine output, and special care for Platosin-treated patients
- Bone marrow function: Requires monitoring of peripheral blood counts and precautions for severe thrombocytopenia and leukopenia
- Hearing function: May produce cumulative ototoxicity, requires audiometry prior to therapy and repeated audiograms
- CNS functions: Induces neurotoxicity, necessitates neurologic examination and observation for anaphylactoid reactions
- Nausea and Vomiting: Marked nausea and vomiting may occur
- Carcinogenicity: Potential for secondary malignancies
- Dental: Increased incidence of microbial infection and delayed healing
Overdose Effects
May result in kidney failure, liver failure, deafness, ocular toxicity, myelosuppression, intractable nausea and vomiting, and death
