Description
Mycophenolate mofetil is an immunosuppressive agent that inhibits inosine monophosphate dehydrogenase, preventing acute rejection of kidney and heart allotransplantation.
Mechanism of Action
Inhibitor of inosine monophosphate dehydrogenase, preventing de novo pathway of guanosine nucleotide synthesis
Pharmacodynamics
Inhibits synthesis of guanosine nucleotides necessary for DNA and RNA synthesis in B and T lymphocytes
Dosage & Administration
- Standard Dosage for Prophylaxis of Renal Rejection:
- Adults: 1 g orally twice a day
- Children: 750 mg to 1 g twice daily depending on body surface area
- Standard Dosage for Prophylaxis of Cardiac Rejection:
- Adults: 1.5 g orally twice a day
- Children: No pediatric data available
- Standard Dosage for Prophylaxis of Hepatic Rejection:
- Adults: 1.5 g orally twice a day
- Children: No pediatric data available
- Standard Dosage for Treatment of First or Refractory Renal Rejection:
- Adults: 1.5 g orally twice a day

- Children: 300 mg/m^2 twice a day
- Usage: Used in combination with corticosteroids
Interaction
Caution with concomitant administration of antacids, azathioprine, cyclosporin, rifampin, sevelamer, cholestyramine, acyclovir, metronidazole, hormonal contraceptives
Contraindications
Hypersensitivity to mycophenolic acid or excipients, pregnancy, breastfeeding

- Diarrhea
- Leukopenia
- Sepsis
- Gastrointestinal candidiasis
- Urinary tract infection
- Herpes simplex
- Vomiting
- Increased frequency of certain types of infections
Pregnancy & Lactation
Contraindicated in pregnancy and breastfeeding, safety during labor and delivery not established
Precautions & Warnings
- Can increase viral infections
- May cause blood dyscrasias
- Can cause serious GI tract complications
- May increase the risk of developing lymphomas and other malignancies
- Avoid live vaccines
- Patients should not donate blood during therapy
Use in Special Populations
Geriatric patients may be at increased risk of adverse events
Overdose Effects
Could result in over suppression of the immune system, increased susceptibility to infections, and bone marrow suppression
