Indications
- Adjuvant treatment of postmenopausal women with hormone receptor positive early breast cancer
- First-line treatment in postmenopausal women with hormone-dependent advanced breast cancer
- Treatment of advanced breast cancer in women with postmenopausal status
- Pre-operative therapy in postmenopausal women with localized hormone receptor positive breast cancer
- Allows subsequent breast-conserving surgery in women not originally considered candidates for this type of surgery
Pharmacology
- Mechanism of Action: Potent and highly specific nonsteroidal aromatase inhibitor
- Effect on Estrogen Biosynthesis in Postmenopausal Women: Suppression of estrogen biosynthesis in all tissues
- Pharmacokinetics:
- Absorption: Rapid and complete absorption from the gastrointestinal tract
- Plasma Protein Binding: Approximately 60%, mainly to albumin
- Metabolic Clearance: Slow relative to hepatic blood flow
- Terminal Elimination Half-Life: About 2 days
Dosage & Administration
- Recommended Dose for Adult and Elderly Patients: 2.5 mg once daily
- Duration of Treatment: 5 years in adjuvant and extended adjuvant setting
- No Dose Adjustment Required for Elderly Patients: 1
- Dosage in Patients with Hepatic and/or Renal Impairment: No adjustment required for patients with renal impairment
Interaction
No clinically significant drug reactions observed with cimetidine and warfarin
Contraindications
- Known or suspected hypersensitivity to letrozole or other aromatase inhibitors
- Pregnancy, lactation, and pre-menopausal women
- Severe hepatic dysfunction

- Excretion in Human Milk: Not known, should not be administered to nursing women
Precautions & Warnings
- No dosage adjustment necessary in breast cancer patients with moderate hepatic dysfunction
- Renal impairment (calculated creatinine clearance: 20 to 50 ml/min) did not affect steady-state plasma letrozole concentration
- Possible fatigue and dizziness
- Caution in operating machinery or driving a car
