Indications
- Adjuvant treatment of postmenopausal women with hormone receptor positive early breast cancer
- First-line treatment in postmenopausal women with hormone-dependent advanced breast cancer
- Treatment of advanced breast cancer in women with natural or artificially induced postmenopausal status
- Pre-operative therapy in postmenopausal women with localized hormone receptor positive breast cancer
Pharmacology
- Potent and highly specific nonsteroidal aromatase inhibitor
- Reduction of estrogen biosynthesis in all tissues
- Suppression of estrogen biosynthesis in peripheral and malignant tissues
- Suppression of serum oestrone and oestradiol by 75-78%
- Plasma protein binding of letrozole is approximately 60%
Dosage & Administration
- Recommended dose: 2.5 mg once daily
- Treatment duration: 5 years in adjutant and extended adjutant setting
- No dose adjustment for elderly patients
- No dosage adjustment required for patients with hepatic impairment or renal impairment
Interaction
- Co-administration with cimetidine and warfarin
Contraindications
- Known or suspected hypersensitivity to letrozole or other aromatase inhibitors
- Contraindicated during pregnancy, lactation, and in pre-menopausal women
- Contraindicated in severe hepatic dysfunction
Side Effects
- Common: Hot flushes, increased level of cholesterol, fatigue, gastrointestinal disorders
- Rare: Nervous disorders, eye disorders, skin disorders, weight decrease, urinary tract infection
Pregnancy & Lactation
- Teratogenicity and maternal toxicity in pregnant rats
- Not recommended for use in pregnant or nursing women
Precautions & Warnings
- No dosage adjustment for breast cancer patients with moderate hepatic dysfunction
- No dose adjustment necessary for renal impairment (calculated creatinine clearance: 20 to 50 ml/min)
- Caution recommended for patients with fatigue and dizziness
Overdose Effects
- No clinical experience of overdosage
- No specific antidote
- Dialysis may be helpful
