Indications
- Adjuvant treatment of postmenopausal women with hormone receptor positive early breast cancer
- First-line treatment in postmenopausal women with hormone-dependent advanced breast cancer
- Pre-operative therapy in postmenopausal women with localized hormone receptor positive breast cancer
- Subsequent treatment after surgery should be in accordance with standard of care
Pharmacology
- Potent and highly specific nonsteroidal aromatase inhibitor
- Mechanism of action involves competitive binding to the haem of the cytochrome P450 subunit
- Reduction of estrogen biosynthesis in all tissues
- Suppression of estrogen biosynthesis in peripheral and malignant tissues
- Rapid and complete absorption from the gastrointestinal tract
- Maximum suppression of estrogen biosynthesis achieved in 48-78 hours
- Steady-state levels reached within 2 to 6 weeks after daily administration
Dosage & Administration
- Recommended dose of 2.5 mg once daily
- Treatment should continue for 5 years in adjutant and extended adjutant setting
- No dose adjustment required for elderly patients
- No dosage adjustment required for patients with hepatic and/or renal impairment
- Not applicable to children
Interaction
- Co-administration with cimetidine and warfarin does not result in clinically significant drug reactions
Contraindications
- Known or suspected hypersensitivity to letrozole or other aromatase inhibitors
- Contraindicated during pregnancy, lactation, and in pre-menopausal women
- Contraindicated in severe hepatic dysfunction
Side Effects
- Common: hot flushes, increased cholesterol level, fatigue, increased sweating, joint pain, skin rash, headache, gastrointestinal disorders, osteoporosis, swelling, depression, weight fluctuations, hypertension, dry skin, vaginal bleeding
- Rare: nervous disorders, carpal tunnel syndrome, impaired sensation, eye irritation, itching, weight decrease, urinary tract infection, elevated enzyme levels
Pregnancy & Lactation
- Teratogenicity and maternal toxicity observed in pregnant rats
- Not recommended for use in pregnant women or nursing women due to lack of adequate studies and potential excretion in human milk
Precautions & Warnings
- Caution recommended in breast cancer patients with moderate hepatic dysfunction
- Renal impairment did not affect steady-state plasma letrozole concentration
- Potential risks and benefits should be carefully considered before prescribing letrozole
- Patients should be advised about potential fatigue and dizziness
Overdose Effects
- No clinical experience of overdosage
- No specific antidote to letrozole
- Dialysis may be helpful due to low protein binding
- Emesis may be induced if the patient is alert
- Supportive care and frequent monitoring of vital signs is appropriate
