Description
- Dopamine antagonist medication for treating nausea and vomiting
Dosage & Administration
- Should be taken 15-30 minutes before meals
- Adults: 10-20 mg (1-2 tablet or 10-20 ml suspension), every 6-8 hours daily
- Children: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml paediatric drops/10 kg body weight, every 6-8 hours daily
Indications
- Dyspeptic symptom complex
- Acute nausea and vomiting
- Parkinson's disease
- Radiological studies
Pharmacology
- Dopamine antagonist action
- Blocks dopamine receptors
- Restores normal motility and tone of the upper gastrointestinal tract
Interaction
- Concomitant administration of anticholinergic drugs may antagonize the antidyspeptic effect
- Antacids and antisecretory drugs should not be given simultaneously
- Main metabolic pathway through CYP3A4
- In vitro data suggest concomitant use of drugs that significantly inhibit CYP3A4
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage, mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Extrapyramidal phenomena
- Increase in plasma prolactin level
- Rare allergic reactions
Pregnancy & Lactation
- Should only be used during the first trimester if justified by therapeutic benefit
- Excreted in breast milk of lactating rats
- Domperidone concentrations in breast milk is 4 times lower than corresponding plasma concentrations
Precautions & Warnings
- Use with absolute caution in children
- Increased risk of extra-pyramidal reactions in young children
- Use with caution in patients with hepatic impairment
Use in Special Populations
- Use in infants with great caution and under close medical supervision
- Use with caution in patients with hepatic impairment
- Dosing frequency should be reduced in patients with severe renal insufficiency
Overdose Effects
- Symptoms may include drowsiness, disorientation and extrapyramidal reactions
- Administration of activated charcoal is recommended
