Description

  • Dopamine antagonist medication for treating nausea and vomiting

Dosage & Administration

  • Should be taken 15-30 minutes before meals
  • Adults: 10-20 mg (1-2 tablet or 10-20 ml suspension), every 6-8 hours daily
  • Children: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml paediatric drops/10 kg body weight, every 6-8 hours daily

Indications

  • Dyspeptic symptom complex
  • Acute nausea and vomiting
  • Parkinson's disease
  • Radiological studies

Pharmacology

  • Dopamine antagonist action
  • Blocks dopamine receptors
  • Restores normal motility and tone of the upper gastrointestinal tract

Interaction

  • Concomitant administration of anticholinergic drugs may antagonize the antidyspeptic effect
  • Antacids and antisecretory drugs should not be given simultaneously
  • Main metabolic pathway through CYP3A4
  • In vitro data suggest concomitant use of drugs that significantly inhibit CYP3A4

Contraindications

  • Known hypersensitivity to the drug
  • Neonates
  • Gastrointestinal hemorrhage, mechanical obstruction or perforation
  • Prolactin releasing pituitary tumor (prolactinoma)

Side Effects

  • Transient intestinal cramps
  • Extrapyramidal phenomena
  • Increase in plasma prolactin level
  • Rare allergic reactions

Pregnancy & Lactation

  • Should only be used during the first trimester if justified by therapeutic benefit
  • Excreted in breast milk of lactating rats
  • Domperidone concentrations in breast milk is 4 times lower than corresponding plasma concentrations

Precautions & Warnings

  • Use with absolute caution in children
  • Increased risk of extra-pyramidal reactions in young children
  • Use with caution in patients with hepatic impairment

Use in Special Populations

  • Use in infants with great caution and under close medical supervision
  • Use with caution in patients with hepatic impairment
  • Dosing frequency should be reduced in patients with severe renal insufficiency

Overdose Effects

  • Symptoms may include drowsiness, disorientation and extrapyramidal reactions
  • Administration of activated charcoal is recommended
