Indications
- Dyspeptic symptom complex

Pharmacology
- Dopamine antagonist
- ChemoreceptorTrigger Zone (CTZ) blocker
- Gastroprokinetic action
- Weak penetration of blood-brain barrier
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
Dosage & Administration
- Should be taken 15-30 minutes before meals
- Oral dose for Adults: 10-20 mg every 6-8 hours daily
- Oral dose for Children: 2-4 ml suspension/10 kg body weight every 6-8 hours daily
- Rectal dose for Adults: 30-60 mg every 4-8 hours
- Maximum dose of 80 mg daily
- Maximum period of treatment is 12 weeks
Interaction
- Anticholinergic drugs antagonize effect
- Antacids and antisecretory drugs lower oral bioavailability
- Main metabolic pathway through CYP3A4
- In vitro use of drugs that significantly inhibit CYP3A4
- Potential influence on absorption of orally administered drugs
- No influence on neuroleptics
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena in young children and adults
- Increase in plasma prolactin level
- Neuroendocrinological phenomena in rare cases
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Use only during the first trimester if justified by therapeutic benefit
- No evidence of increased risk of malformations in humans
- Excreted in breast milk at lower concentrations than plasma
- Nursing not recommended unless expected benefits outweigh potential risk
Precautions & Warnings
- Caution in children due to risk of extra-pyramidal reactions
- Caution in patients with hepatic impairment

- Caution in infants due to immature blood-brain barrier
- Caution in patients with hepatic impairment
- Dose adjustments in patients with severe renal insufficiency
- Regular reviews for patients on prolonged therapy
Overdose Effects
- Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
- Administration of activated charcoal recommended
- Anticholinergic, antiparkinson drugs may be helpful in controlling extrapyramidal reactions
