Indications
- Sole anesthetic agent for brief procedures
- Induction of anesthesia prior to other anesthetic agents

Composition
- Thiopental 500 mg injection
- Thiopental 1 gm injection
- Sterile mixture of Thiopental Sodium and anhydrous Sodium Carbonate
- Ampoule of 10 ml Water for Injection
Pharmacology
- Ultra-short-acting depressant of the central nervous system
- Induces hypnosis and anesthesia
- Binds at a distinct site associated with a Cl- ionopore at the GABA receptor
- Prolongs the post-synaptic inhibitory effect of GABA in the thalamus
Dosage & Administration
- Administered intravenously
- Normally as a 2.5% w/v solution
- Premedication with atropine or scopolamine
- Test dose of 25 mg to 75 mg to assess tolerance
- Normal dosage for induction of anesthesia
- Different dosages for adult, children, and elderly patients
Interaction
- Possible increase in difficulty in producing anesthesia with alcohol or CNS depressants
- Additive action with other CNS depressants
- Increased hypotension and excitatory effects with phenothiazine antipsychotics
Contraindications
- Absence of suitable veins for intravenous administration
- Hypersensitivity to barbiturates
- Variegate porphyria or acute intermittent porphyria
- Status asthmaticus
- Severe cardiovascular disease
- Hypotension or shock
Side Effects
- Respiratory depression
- Myocardial depression
- Cardiac arrhythmias
- Prolonged somnolence and recovery
- Hypotension
- Tachycardia
- Sneezing, coughing, bronchospasm
- Laryngospasm and shivering
- Anaphylactic reactions
- Urticaria, bronchospasm, vasodilation, and edema
Pregnancy & Lactation
- Pregnancy Category C
- Not known whether it can cause fetal harm
- Crosses the placental barrier
- May appear in the milk of nursing mothers
Precautions & Warnings
- Requires constant attendance and facilities for support
- Administered with caution in patients with preexisting hypotension
- May be habit-forming
- Resuscitative and endotracheal intubation equipment should be readily available
Overdose Effects
- Too rapid or repeated administration may lead to overdosage
- Alarming fall in blood pressure
- Apnoea, laryngospasm, and respiratory difficulties

Storage Conditions
- Store at controlled room temperature of 15°C to 30°C
- Solutions should be freshly prepared and used immediately
- Any remaining portion should be discarded
