Indications
- Sole anesthetic agent for brief procedures
- Induction of anesthesia prior to other anesthetic agents
- Supplementing regional anesthesia
- Providing hypnosis during balanced anesthesia
- Control of convulsive states
- Neurosurgical patients with increased intracranial pressure

Composition
- Thiopental 500 mg injection
- Thiopental 1 gm injection
- Sterile mixture of Thiopental Sodium
- Anhydrous Sodium Carbonate as a buffer
- Water for Injection
Pharmacology
- Ultrashort-acting depressant of the central nervous system
- Induces hypnosis and anesthesia, but not analgesia
- Binds at a distinct binding site associated with a Cl- ionopore at the GABA receptor
- Increases the duration of time for which the Cl- ionopore is open
Dosage & Administration
- Administered intravenously as a 2.5% w/v solution
- Can also be administered as a 5% w/v solution
- Different dosages for adults, children, and elderly
- Peak effect of premedication should be reached shortly before induction
Interaction
- Possible increase in difficulty in producing anaesthesia with alcohol or CNS depressants
- Additive action with other CNS depressants
- Increased hypotension and excitatory effects with phenothiazine antipsychotics
- Decreased requirement of TPS with certain drugs
Contraindications
- Absence of suitable veins for intravenous administration
- Hypersensitivity to barbiturates
- Variegate porphyria or acute intermittent porphyria
- Severe cardiovascular disease
- Hypotension or shock
- Conditions prolonging or potentiating the hypnotic effect
Side Effects
- Respiratory depression
- Myocardial depression
- Cardiac arrhythmias
- Prolonged somnolence and recovery
- Hypotension
- Tachycardia
- Anaphylactic reactions
- Urticaria, bronchospasm, vasodilation, and edema
Pregnancy & Lactation
- Pregnancy Category C
- Not recommended unless clearly needed
- Crosses placental barrier and may appear in nursing mothers' milk
Precautions & Warnings
- Should be administered with caution to patients with preexisting hypotension
- May be habit forming
- Adequate facilities for respiration and circulation should be available
- Only persons qualified in the use of anesthetics should administer TPS
Overdose Effects
- Too rapid or repeated administration may lead to overdosage
- Alarming fall in blood pressure
- Apnoea, laryngospasm, and respiratory difficulties

Storage Conditions
- Store at controlled room temperature of 15°C to 30°C
- Solutions should be freshly prepared and used immediately
- Any portion of the contents remaining should be discarded
