Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Decreasing eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol
  • Inhibits synthesis of prostaglandins
  • Biological activity associated with S-form
  • Pharmacokinetic properties: Linear, highly protein bound, mostly metabolized in liver

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Inhibits cyclooxygenase enzyme system and prostaglandin synthesis
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet: Recommended dose of 10 mg every 4-6 hours for short-term use (up to 7 days)
  • Injection: Single or multiple doses for moderately severe, acute pain management, with specific recommended doses based on age, renal function, and body weight
  • Eye Drops: Recommended dosage of 1 drop in each eye 4 times daily

Interaction

  • Other NSAIDs or aspirin: Increases side effects
  • Anti-coagulants: Enhances anti-coagulant effect
  • Beta Blocker: Reduces anti-hypertensive effect
  • ACE Inhibitors: Increases risk of renal impairment
  • Methotrexate: Enhances toxicity of methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic before surgery

Side Effects

  • Nausea, vomiting, gastro-intestinal bleeding, melena, peptic ulcer, pancreatitis, anxiety, drowsiness, headache, excessive thirst, fatigue, bradycardia, hypertension, palpitation, chest pain, infertility in females, pulmonary edema

Precautions & Warnings

  • Caution in patients over 65 years
  • Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Eye Drops: Discontinue use with evidence of corneal epithelial breakdown, avoid while wearing contact lenses
