Indications
- Short-term management of moderate to severe acute post-operative pain
- Relieves pain, inflammation, and light sensitivity after ocular surgery
- Decreases eye inflammation or redness caused by seasonal allergies
Description
- Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
- Chemically known as 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
- Inhibits synthesis of prostaglandins and acts as a peripherally acting analgesic
- Highly protein bound and largely metabolized in the liver
Pharmacology
- Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
- Acts by inhibiting the cyclooxygenase enzyme system and hence inhibits prostaglandin synthesis
- Demonstrates minimal anti-inflammatory effect at its analgesic dose
Dosage & Administration
- Tablet recommended dose: 10 mg every 4-6 hours for short-term use (up to 7 days)
- Injection for moderate to severe acute pain, with specific doses for different age groups and conditions
- Eye drops: 1 drop in each eye 4 times daily for adults
Interaction
- NSAIDs or aspirin increase the side effects of Analac
- Anti-coagulants enhance anti-coagulant effect
- Beta Blockers reduce the anti-hypertensive effect
- ACE Inhibitors increase the risk of renal impairment
- Methotrexate enhances the toxicity of methotrexate
Contraindications
- Hypersensitivity to Ketorolac or other NSAIDs
- Use in children under 16 years of age
- Prophylactic analgesic before surgery
Side Effects
- Nausea, vomiting, gastro-intestinal bleeding, melana, peptic ulcer, pancreatitis
- Anxiety, drowsiness, headache, excessive thirst, fatigue, bradycardia, hypertension
- Palpitation, chest pain, infertility in females, pulmonary edema
Pregnancy & Lactation
- Ketorolac Tromethamine is FDA Pregnancy category C
- Avoid use in pregnancy and lactation unless benefits outweigh risks
Precautions & Warnings
- Exercise caution in patients over 65 years
- Risk factors include active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
- Monitor patients with evidence of corneal epithelial breakdown when using topical NSAIDs
- Avoid administration while wearing contact lenses for the ophthalmic solution
