Indications

  • Short-term management of moderate to severe post-operative pain

Description

  • Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical composition: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits prostaglandin synthesis
  • Biological activity associated with the S-form
  • Linear pharmacokinetic property
  • Highly protein bound
  • Metabolized in the liver
  • Metabolites and some unchanged drugs excreted in urine

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Inhibits cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Recommended tablet dose: 10 mg every 4-6 hours for up to 7 days, not for chronic use
  • Recommended injection dose: may be used as single or multiple doses for moderately severe, acute pain that requires analgesia at the opioid level
  • Onset of analgesic effect within 30 minutes for IV or IM administration
  • Duration of analgesic effect usually 4 to 6 hours
  • Specific dosing instructions for different patient groups

Interaction

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE inhibitors
  • Enhanced toxicity of methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic before surgery

  • Discontinue use of topical NSAIDs in patients with evidence of corneal epithelial breakdown
  • Avoid administering while wearing contact lenses
