Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Decrease eye inflammation or redness caused by seasonal allergies

Description

  • Pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibition of prostaglandin synthesis
  • Biological activity associated with the S-form
  • Linear pharmacokinetic property
  • Highly protein bound
  • Metabolized in the liver
  • Products of metabolism and some unchanged drugs excreted in the urine

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Inhibition of the cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet recommended dose: 10 mg every 4-6 hours for short-term use
  • Injection recommended dose: Various dosing based on age, weight, and renal function
  • IV bolus administration over at least 15 seconds
  • IM administration given slowly and deeply into the muscle
  • Duration of analgesic effect: usually 4 to 6 hours
  • Eye Drops: Adults: 1 drop in each eye 4 times daily

Interaction

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anticoagulant effect with anticoagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE inhibitors
  • Enhanced methotrexate toxicity with methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Use in children under 16 years of age
  • Prophylactic analgesic use before surgery

Precautions & Warnings

  • Caution in patients over 65 years
  • Caution in patients with active or suspected peptic ulcer or gastrointestinal bleeding, asthma, and liver dysfunction
  • Discontinue use of topical NSAIDs in patients with corneal epithelial breakdown and monitor corneal health
  • Avoid administration while wearing contact lenses
