Pharmacology
- Dopamine antagonist
- Gastroprokinetic action
- Blocks dopamine receptors
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates lower esophageal sphincter pressure
Dosage administration
- To be taken 15-30 minutes before meals
- Maximum dose of 80 mg daily for adults
- Different dosage for dyspeptic symptoms and acute/sub-acute conditions
- Rectal administration in suppositories for adults and children
Interaction
- Antagonistic effect with anticholinergic drugs
- Lowered bioavailability with antacids and antisecretory drugs
- Metabolism through CYP3A4
- Potential inhibition by azole antifungals, macrolide antibiotics, HIV protease inhibitors, nefazodone, etc.
- Influence on the absorption of concomitantly orally administered drugs
- No influence on neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to domperidone
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena in young children and adults
- Increase in plasma prolactin level
- Possible neuroendocrinological phenomena such as galactorrhoea and gynaecomastia
- Rare allergic reactions such as rash and urticaria
Pregnancy lactation
- Use only during the first trimester of pregnancy if justified by anticipated therapeutic benefit
- No evidence of increased risk of malformations in humans
- Excretion in breast milk of lactating rats
- Not recommended during nursing unless benefits outweigh potential risks
- No effect on mental alertness
Precautions warnings
- Increased risk of extra-pyramidal reactions in young children
- Caution in patients with hepatic impairment
- Caution in infants due to immature blood-brain barrier
- Reduced dosing frequency in patients with severe renal insufficiency

Overdose effects
- Symptoms include drowsiness, disorientation, and extrapyramidal reactions
