Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relieves pain, inflammation, and light sensitivity after ocular surgery
  • Decreases eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical name is 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits synthesis of prostaglandins
  • Biological activity associated with the S-form
  • Highly protein bound
  • Metabolized in the liver

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Inhibits the cyclooxygenase enzyme system
  • Demonstrates minimal anti-inflammatory effect at its analgesic dose

Dosage & Administration

  • Tablet recommended dose of 10 mg every 4-6 hours for short-term use
  • Tablet not recommended for chronic use or doses exceeding 40 mg/day
  • Injection may be used for moderately severe, acute pain
  • IV bolus must be given over no less than 15 seconds
  • Maximum analgesic effect seen within 1 to 2 hours after dosing
  • Duration of analgesic effect is usually 4 to 6 hours
  • Eye drops recommended 1 drop in each eye 4 times daily

Interaction

  • Other NSAIDs or aspirin can increase side effects
  • Anti-coagulants can enhance anti-coagulant effect
  • Beta Blockers can reduce the anti-hypertensive effect
  • ACE Inhibitors can increase the risk of renal impairment
  • Methotrexate can enhance the toxicity of methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Not to be used in children under 16 years of age
  • Contraindicated as prophylactic analgesic before surgery

Precautions & Warnings

  • Caution in patients over the age of 65 years
  • Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Topical NSAIDs should be discontinued in patients with evidence of corneal epithelial breakdown
  • Eye drops should not be administered while wearing contact lenses
