Indications

  • Management of acute post-operative pain
  • Relief of pain after ocular surgery
  • Reduction of eye inflammation caused by allergies

Description

  • Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol
  • Inhibits synthesis of prostaglandins
  • Biological activity associated with the S-form
  • Highly protein bound
  • Metabolized in the liver
  • Metabolites and some unchanged drug excreted in urine

Pharmacology

  • Potent analgesic
  • Acts by inhibiting cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet: recommended dose is 10 mg every 4-6 hours for short-term use
  • Injection: may be used for moderately severe, acute pain management
  • Injection: IV bolus must be given over no less than 15 seconds
  • Injection for adult IM dosing: one dose of 60 mg for <65 years, 30 mg for >65 years or renally impaired and/or <50 kg
  • Injection for adult IV dosing: one dose of 30 mg for <65 years, 15 mg for >65 years or renally impaired and/or <50 kg
  • Injection for 2 to 16 years old: IM dosing one dose of 1 mg/kg up to a maximum of 30 mg, IV dosing one dose of 0.5 mg/kg up to a maximum of 15 mg
  • Eye Drops: Adults: 1 drop in each eye 4 times daily

Interaction

  • Other NSAIDs or aspirin increase the side effects of Ketonic
  • Anti-coagulants enhance anti-coagulant effect
  • Beta Blocker reduce the anti-hypertensive effect
  • ACE Inhibitors increase the risk of renal impairment
  • Methotrexate enhance the toxicity of methotrexate

Contraindications

  • Hypersensitivity to the drug or other NSAIDs
  • Use in children under 16 years of age
  • Prophylactic analgesic before surgery

  • Eye Drops: avoid in patients with corneal epithelial breakdown
  • Eye Drops: avoid use with contact lenses
