Indications
- Management of acute post-operative pain

Description
- Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
- Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol
- Inhibits synthesis of prostaglandins
- Biological activity associated with the S-form
- Highly protein bound
- Metabolized in the liver
- Metabolites and some unchanged drug excreted in urine
Pharmacology
- Potent analgesic
- Acts by inhibiting cyclooxygenase enzyme system
- Minimal anti-inflammatory effect at analgesic dose
Dosage & Administration
- Tablet: recommended dose is 10 mg every 4-6 hours for short-term use
- Injection: may be used for moderately severe, acute pain management
- Injection: IV bolus must be given over no less than 15 seconds
- Injection for adult IM dosing: one dose of 60 mg for <65 years, 30 mg for >65 years or renally impaired and/or <50 kg
- Injection for adult IV dosing: one dose of 30 mg for <65 years, 15 mg for >65 years or renally impaired and/or <50 kg
- Injection for 2 to 16 years old: IM dosing one dose of 1 mg/kg up to a maximum of 30 mg, IV dosing one dose of 0.5 mg/kg up to a maximum of 15 mg
- Eye Drops: Adults: 1 drop in each eye 4 times daily
Interaction
- Other NSAIDs or aspirin increase the side effects of Ketonic
- Anti-coagulants enhance anti-coagulant effect
- Beta Blocker reduce the anti-hypertensive effect
- ACE Inhibitors increase the risk of renal impairment
- Methotrexate enhance the toxicity of methotrexate
Contraindications
- Hypersensitivity to the drug or other NSAIDs
- Use in children under 16 years of age
- Prophylactic analgesic before surgery
