Indications

  • short-term management of moderate to severe acute post-operative pain
  • relieves pain, inflammation, and light sensitivity after ocular surgery
  • decreases eye inflammation or redness caused by seasonal allergies

Description

  • belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
  • chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • inhibits synthesis of prostaglandins
  • S-form is associated with the biological activity
  • highly protein bound
  • largely metabolized in liver
  • metabolites and some unchanged drugs excreted in urine

Pharmacology

  • potent analgesic
  • inhibits cyclooxygenase enzyme system
  • minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • 10 mg tablet every 4-6 hours for up to 7 days
  • Injection: single or multiple doses, regular or when necessary schedule
  • IV bolus to be given over no less than 15 seconds
  • analgesic effect begins within 30 minutes
  • maximum effect in 1 to 2 hours after dosing IV or IM
  • duration of analgesic effect usually 4 to 6 hours
  • specific dosages for different age groups and conditions

Interaction

  • Other NSAIDs or aspirin: Increase side effects of Kflam
  • Anti-coagulants: Enhance anti-coagulant effect
  • Beta Blocker: Reduce anti-hypertensive effect
  • ACE Inhibitors: Increase risk of renal impairment
  • Methotrexate: Enhance toxicity of methotrexate

Contraindications

  • hypersensitivity to Ketorolac or other NSAIDs
  • not to be used in children under 16 years of age
  • contraindicated as prophylactic analgesic before surgery

Precautions & Warnings

  • exercise caution in patients over 65 years
  • caution in patients with active or suspected peptic ulcer or gastrointestinal bleeding
  • caution in patients with asthma and liver dysfunction
  • discontinue topical NSAIDs usage if evidence of corneal epithelial breakdown is found
  • do not administer while wearing contact lenses
