Indications
- Short-term management of moderate to severe acute post-operative pain

Description
- Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
- Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
- Inhibits synthesis of prostaglandins
- Peripherally acting analgesic
- Biological activity associated with the S-form
- Pharmacokinetic property is linear
- Highly protein bound
- Metabolized in the liver
- Metabolites and some unchanged drugs are excreted in the urine
Pharmacology
- Potent analgesic within the non-steroidal anti-inflammatory drugs (NSAIDs) class
- Inhibits the cyclooxygenase enzyme system
- Minimal anti-inflammatory effect at analgesic dose
Dosage & Administration
- Recommended tablet dose: 10mg every 4-6 hours for short-term use (up to 7 days)
- Tablet doses exceeding 40mg/day not recommended
- Injection for management of moderately severe acute pain, single or multiple doses
- IV bolus administration over at least 15 seconds
- IM administration should be given slowly and deeply into the muscle
- Analgesic effect begins within 30 minutes with maximum effect in 1 to 2 hours after dosing IV or IM
- Duration of analgesic effect is usually 4 to 6 hours
- Specific dosing guidelines for different age groups and conditions
- 30mg Ketorolac injection every 6 hours for patients under 65 years of age
- 15mg Ketorolac injection every 6 hours for elderly and renally impaired patients
- Additional guidance for conversion from parenteral to oral therapy
- Specific dosing guidance for eye drops
Interaction
- Increased side effects with other NSAIDs or aspirin
- Enhanced anti-coagulant effect with anti-coagulants
- Reduced anti-hypertensive effect with beta blockers
- Increased risk of renal impairment with ACE Inhibitors
- Enhanced toxicity of methotrexate
Contraindications
- Hypersensitivity to Ketorolac or other NSAIDs
- Not to be used in children under 16 years of age
- Contraindicated as prophylactic analgesic before surgery
