Indications

  • Management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Reduction of eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
  • Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits synthesis of prostaglandins
  • Peripherally acting analgesic
  • Highly protein bound
  • Metabolized in the liver
  • Metabolites and some unchanged drug excreted in urine

Pharmacology

  • Inhibits cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet: Recommended dose of 10 mg every 4-6 hours for short-term use (up to 7 days)
  • Injection: Single or multiple doses for postoperative pain; IV bolus must be given over at least 15 seconds
  • Eye Drops: 1 drop in each eye 4 times daily
  • Specific dosing for different age groups and conditions

Interaction

  • Increase in side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduction in anti-hypertensive effect with Beta Blocker
  • Increased risk of renal impairment with ACE Inhibitors
  • Enhanced toxicity of methotrexate

Contraindications

  • Hypersensitivity to the drug or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic before surgery

Precautions & Warnings

  • Caution in patients over 65 years old
  • Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Eye Drops: Discontinue if evidence of corneal epithelial breakdown; should not be used while wearing contact lenses
