Indications

  • Management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Decrease eye inflammation or redness caused by seasonal allergies

Description

  • Pyrrolo-pyrrole group nonsteroidal anti-inflammatory drug (NSAID)
  • Chemical composition: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits synthesis of prostaglandins
  • Associated with the S-form
  • Linear pharmacokinetic property
  • Highly protein bound
  • Metabolized in the liver
  • Excretion in the urine

Pharmacology

  • Inhibits cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet recommended dose: 10 mg every 4-6 hours for short-term use
  • Injection recommended in postoperative setting
  • Different dosing for different age groups and conditions
  • IM and IV administration
  • Duration of analgesic effect: 4 to 6 hours
  • Eye Drops: 1 drop in each eye 4 times daily

Interaction

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE inhibitors
  • Enhanced methotrexate toxicity

Contraindications

  • Hypersensitivity to this drug or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic before surgery

Precautions & Warnings

  • Caution in patients over 65 years
  • Caution in patients with peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Eye Drops: Discontinue use with corneal epithelial breakdown, monitoring for corneal health, not to be administered with contact lenses
