Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relieves pain, inflammation, and light sensitivity after ocular surgery
  • Decreases eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
  • Inhibits synthesis of prostaglandins as a peripherally acting analgesic
  • Biological activity associated with the S-form
  • Pharmacokinetic property is linear
  • Highly protein bound and largely metabolized in the liver

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Inhibits the cyclooxygenase enzyme system and prostaglandin synthesis
  • Demonstrates minimal anti-inflammatory effect at its analgesic dose

Dosage & Administration

  • Recommended tablet dose: 10 mg every 4-6 hours for short-term use
  • Injection used for moderately severe acute pain, single or multiple doses, regular or when necessary schedule
  • Eye drops recommended dose: 1 drop in each eye 4 times daily
  • Different doses for patients of different ages and conditions

Interaction

  • Use with other NSAIDs or aspirin increases side effects
  • Interacts with anti-coagulants, beta blockers, ACE inhibitors, and methotrexate

Contraindications

  • Contraindicated in patients hypersensitive to the drug or other NSAIDs
  • Should not be used in children under 16 years of age
  • Contraindicated as prophylactic analgesic before surgery

Side Effects

  • Nausea, vomiting, gastrointestinal bleeding, melena, peptic ulcer, pancreatitis
  • Anxiety, drowsiness, headache, excessive thirst, fatigue
  • Bradycardia, hypertension, palpitation, chest pain, infertility in females, pulmonary edema

Pregnancy & Lactation

  • US FDA Pregnancy category C: Avoid in pregnancy and lactation unless benefits outweigh risks to the fetus

Precautions & Warnings

  • Caution in patients over 65 years, active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Eye drops caution: Discontinue use with corneal epithelial breakdown, avoid use with contact lenses
