Indications
- Short-term management of moderate to severe acute post-operative pain
- Relieves pain, inflammation, and light sensitivity after ocular surgery
- Decreases eye inflammation or redness caused by seasonal allergies
Description
- Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
- Inhibits synthesis of prostaglandins as a peripherally acting analgesic
- Biological activity associated with the S-form
- Pharmacokinetic property is linear
- Highly protein bound and largely metabolized in the liver
Pharmacology
- Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
- Inhibits the cyclooxygenase enzyme system and prostaglandin synthesis
- Demonstrates minimal anti-inflammatory effect at its analgesic dose
Dosage & Administration
- Recommended tablet dose: 10 mg every 4-6 hours for short-term use
- Injection used for moderately severe acute pain, single or multiple doses, regular or when necessary schedule
- Eye drops recommended dose: 1 drop in each eye 4 times daily
- Different doses for patients of different ages and conditions
Interaction
- Use with other NSAIDs or aspirin increases side effects
- Interacts with anti-coagulants, beta blockers, ACE inhibitors, and methotrexate
Contraindications
- Contraindicated in patients hypersensitive to the drug or other NSAIDs
- Should not be used in children under 16 years of age
- Contraindicated as prophylactic analgesic before surgery
Side Effects
- Nausea, vomiting, gastrointestinal bleeding, melena, peptic ulcer, pancreatitis
- Anxiety, drowsiness, headache, excessive thirst, fatigue
- Bradycardia, hypertension, palpitation, chest pain, infertility in females, pulmonary edema
Pregnancy & Lactation
- US FDA Pregnancy category C: Avoid in pregnancy and lactation unless benefits outweigh risks to the fetus
Precautions & Warnings
- Caution in patients over 65 years, active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
- Eye drops caution: Discontinue use with corneal epithelial breakdown, avoid use with contact lenses
