Indications
- Short-term management of moderate to severe acute post-operative pain
- Relieves pain, inflammation, and light sensitivity after ocular surgery
- Decreases eye inflammation or redness caused by seasonal allergies
Description
- Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
- Chemical composition: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
- Inhibits the synthesis of prostaglandins and acts as a peripherally acting analgesic
- Biological activity associated with the S-form
- Linear pharmacokinetic property
- Highly protein bound
- Metabolized in the liver
- Products of metabolism and some unchanged drugs are excreted in the urine
Pharmacology
- Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
- Inhibits the cyclooxygenase enzyme system and prostaglandin synthesis
- Demonstrates minimal anti-inflammatory effect at analgesic dose
Dosage & Administration
- Tablet recommended dose: 10 mg every 4-6 hours for short-term use only (up to 7 days)
- Injection may be used for the management of moderately severe, acute pain
- Recommended IM and IV doses for different age groups and conditions
- Maximum daily dose limits for different populations
- Eye drops dosage: 1 drop in each eye 4 times daily
Interaction
- Other NSAIDs or aspirin increase the side effects of Rotek
- Anti-coagulants enhance anti-coagulant effect
- Beta Blocker reduces the anti-hypertensive effect
- ACE Inhibitors increase the risk of renal impairment
- Methotrexate enhances the toxicity of methotrexate
Contraindications
- Hypersensitivity to the drug or other NSAIDs
- Not to be used in children under 16 years of age
- Contraindicated as prophylactic analgesic before surgery

Precautions & Warnings
- Exercise caution in patients over the age of 65 years
- Also caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
- Patients with evidence of corneal epithelial breakdown should discontinue use of topical NSAIDs and be closely monitored for corneal health
- Lopadol ophthalmic solution should not be administered while wearing contact lenses
