Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Decrease eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
  • Chemical name: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits synthesis of prostaglandins
  • Biological activity associated with the S-form
  • Pharmacokinetic property is linear
  • Highly protein bound
  • Largely metabolized in the liver
  • Metabolites and some unchanged drugs excreted in the urine

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Acts by inhibiting the cyclooxygenase enzyme system
  • Demonstrates minimal anti-inflammatory effect at its analgesic dose

Dosage & Administration

  • Tablet: Recommended dose is 10 mg every 4-6 hours for up to 7 days
  • Injection: Used for moderately severe, acute pain management postoperatively
  • IV bolus must be given over no less than 15 seconds
  • IM administration given slowly and deeply into the muscle
  • Onset of analgesic effect within 30 minutes
  • Maximum effect in 1 to 2 hours
  • Duration of analgesic effect is usually 4 to 6 hours
  • Various dosing for different patient categories

Interaction

  • Other NSAIDs or aspirin increase the side effects of Ketorolac Tromethamine
  • Anti-coagulants enhance anti-coagulant effect
  • Beta Blocker reduces the anti-hypertensive effect
  • ACE Inhibitors increase the risk of renal impairment
  • Methotrexate enhances the toxicity of methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Not for use in children under 16 years
  • Contraindicated as prophylactic analgesic before surgery

  • Special precautions for use of ophthalmic solution in patients with corneal epithelial breakdown and those wearing contact lenses
